FORMS PROVIDED ARE RECOMMENDED BUT NOT NECESSARILY REQUIRED.  PLEASE VERIFY ACCURACY WITH MDOT OR AIRPORT ATTORNEY PRIOR TO USE.

Forms on this page are recommended for use as they contain language required by Federal and State laws, regulations, and guidelines.  Other forms may be used only after all required language has been included and verified by the FAA or MDOT.  It is recommended that airport sponsors consult an attorney whenever documents are prepared or used for the intended purpose of acquiring property or avigation easement rights.  It is also recommended that the attorney have condemnation experience and/or knowledge of existing Federal and State eminent domain laws.

** A Notice to Quit should be given to the property owner if the owner is being displaced.  The date required to move should correspond to the date on the Relocation Eligibility Notice.
