Do I have to mail my applications?
 
Answer:
Yes, you must mail or fax the forms to the appropriate location listed for each form that you print out. Follow the instructions on the screen and on each form. Also, it is important that you enclose the fee with the appropriate form. Enhancements are planned for the future so that forms can be sent electronically in place of the mail but for now, you must mail or fax the forms.
