This application should be completed by those who will be buying directly from the tobacco products manufacturer, importing into Michigan tobacco products from an unlicensed source, offering tobacco products for resale to other businesses, operating vending machines, acting as a transporter or a transportation company. You will also need to complete form 3797. Complete form 323 if you will be licensed for other tobacco products. |

This application should be completed by persons who acquire non-cigarette tobacco products from licensed manufacturers of tobacco products, or who import non-cigarette tobacco products. The Tobacco Products Act requires a licensed wholesaler or unclassified acquirer to affix the stamp to shipping containers of non-cigarette tobacco products and remit tax to the department upon importation of the products. |

This application should be completed by businesses that purchase aviation fuel from a supplier or another aviation fuel registrant for the purpose of resale. Products identified as aviation fuel may only be purchased, sold, or used for aviation purposes. Certification as an aviation fuel registrant does not exempt the purchaser from the $.03 per gallon excise tax. |
