  1. How do I file a complaint regarding the conduct of a licensed charitable gaming event?

    Only written complaints are recorded and investigated.  A Complaint Record may be obtained from our website,
    or by calling (517) 335-5786.  It is important that a Complaint Record be completed with as much information as possible, including the licensee's name, license number, date and location of the event, and a written explanation of the misconduct.  Mail the completed Complaint Record to the address indicated on the bottom of the form.  If it appears a rule violation may have occurred, the complaint will be assigned to an inspector for investigation.  Complainants will be notified of any decisions only if a name, address, and phone number are provided.
