Raffle FAQs

  1. I received a Workers Service Record and Raffle Ticket Accountability form with my raffle license.  What do I do with these forms?

    Both forms are required to be completed and retained with your game records -- do not send them to Lansing.  If you applied for a large raffle license, you will receive a pre-filled financial statement in addition to the Workers Service Record and Raffle Ticket Accountability form.  The financial statement must be completed and returned to the Charitable Gaming Division by the 10th day of the month following the month of your event; for example, if the raffle was held February 2, then the financial statement is due March 10.
