Project: The Family Links program is based on the Healthy Start community outreach and home-visiting model towards strengthening social and medical services to at-risk families in rural northern Michigan. Intensified home visitation services will be provided to decrease isolation and promote social engagement and linkages of human services to these rural at-risk families.
