Credit Card and Electronic Fund Transfer (EFT) payments for application fees are now being accepted by the LWMD District offices and the LWMD Lansing office.  To mail in payments, use the appropriate form below and mail to the office reviewing your application.  Due to security reasons, facsimile or emailed payment forms can not be accepted at this time.
