The Joint Permit Application (JPA) is now available electronically for on-line submittal through the Michigan Timely Application and Permit Service (MITAPS). In addition to submitting the JPA, the MITAPS provides the option to attach supplemental documents electronically and to pay required fees by credit card. LWMD invites you to submit your next application through this service.

Credit Card and Electronic Funds Transfer (EFT) Payments

Modifying the language and/or instructions on the provided application form could result in rejection of the application, forfeiture of the fee, and possible legal action.

Adobe Reader or Acrobat Version 5 or higher is required to open either document. The latest version of Adobe Reader is available free from  Adobe .  If the form cannot be saved after it opens, right click on the link and select "Save Target As..."  When printing this form, uncheck "Shrink oversized pages to paper size" (Adobe Reader Version 5) or select Paper Scaling: None (Adobe Reader Version 6) in the Print dialogue box so that the application prints at the correct size.

Form fields in the fill-in form can be displayed by checking the "Show background and hover color for form fields" check box under Edit, Preferences, Forms. You will not be able to save information you enter in the form fields unless you have Adobe Acrobat.

For sample drawings, go to Application Appendices. We advise you to refer to, or print out, the Application Appendices for Sample drawings, also located on the main menu, www.michigan.gov/jointpermit. Refer to the application for a listing of sample drawings associated with your project. The location maps, plot plan, and cross-sections, along with the fee (fee schedule) , are required to be included with your application. Additional sample drawings are also available in Appendix B of the Joint Permit Application Training Manual.
