Note: DWRF/SRF Programs Requirement - Disadvantaged Business Enterprises (DBE)
The U.S. Environmental Protection Agency (EPA) has created the DBE program to increase the participation of small, minority and women owned firms in the procurement process for goods and services as required by rule, 40 CFR Part 33. There are substantive changes made by the DBE rule. The changes include certification requirements for minority and women owned businesses, Good Faith Efforts, a bidders list, and contract administration requirements. The DBE program requires information on the participation of minority and women owned businesses be reported to the EPA.

The DBE requirements can be found in the "Required Standard Contract Language for Disadvantaged Business Enterprises (DBE) and Debarment Certification." This boilerplate, including the Debarment Form and EPA Forms 6100-2, 6100-3, and 6100-4, (SRF and DWRF only) must be included in the final contract documents. More detailed clarification can be obtained in the Design Phase Guidance (Section G and Attachment 1).

The tables below list a number of DWRF and SRF/SWQIF forms/guidance that you can either print or download and save to your computer system.

To download, simply double click the hyperlink next to the name of the document you want that indicates the format that you currently want to use on your system. For detailed directions about downloading and using the Microsoft Word 97 format, please see the bottom of this page . To view and/or print a document in Portable Document Format (PDF), Acrobat Reader is required (a free download).

Helpful Tips :

  • If you do not have Microsoft Word 97, use the PDF version of the document.
  • Review the quality of printing of your document and do not submit a document where printing is incomplete, not formatted properly, or otherwise looks odd.
  • To retain a documents proper formatting, document downloads must enable macros to run.
  • At this time, documents which require a signature can not be returned via e-mail.

How to Download and Use RF Forms and Guidance

The following steps will allow you to save RF forms/guidance, for your use, to your computer. The first column shows the form/guidance file name. The second column shows the Microsoft Word 97 version of the document and the third column shows the PDF version of the document.

To save a file to your computer using Microsoft Word 97, find the form/guidance document file you need from the list above and double click on the words "Word 97" in the second column. After you have double clicked on this column, a window may open informing you that you are about to download an item from another server. This warning will vary based on the browser you are using. Once the document opens, you can select the "Save As" command from your "File" menu and save the document to the templates folder (e.g. "C:/Program Files/MSOffice/Templates").

Once you have saved the document to your templates folder, the "Tab" key, or "Up" and "Down" arrow keys, will allow you to move around the document. Where a check box appears in a form, you can choose to mark, or unmark, that box by using the "Space Bar".

If you decide to "Open the file from its current location", do not be alarmed if a "Warning" dialog box pops up indicating, "The document you are opening contains macros or customizations". All of the forms contain macros and/or customizations from a trusted source. Therefore, please select to enable the macros to proceed with opening the document. You can then fill in the form and print or save it directly from the Revolving Loan and Operator Certification Section web site.
