Forms and GuidanceAgency: Environmental Quality
Note: New DWRF/CWSRF Contract Boilerplate New construction contract boilerplate is now available and must be included in all construction contract specifications. The new boilerplate accommodates three distinct program requirements and requires very deliberate actions by the owner and all potential bidders:
- Davis-Bacon Prevailing Federal Wages, Including Labor Standards Provisions: The U.S. Environmental Protection Agency (EPA) fiscal year (FY) 2012 appropriation bill (Public Law 112-74) mandates compliance with Federal prevailing wage requirements for all DWRF and CWSRF loans funded for FY 2012 and each fiscal year thereafter. The owner must obtain a copy of the appropriate Wage Determination and incorporate it into this section of the boilerplate prior to bid advertisement and is required to conduct prescribed compliance activities during construction.
- Disadvantaged Business Enterprises (DBE): The EPA created the DBE program to increase the participation of small, minority and women owned firms in the procurement process for goods and services as required by rule, 40 CFR Part 33. The DBE regulation requires conscious efforts from owners and prospective contruction contractors. There are substantive changes made by the DBE rule. The changes include certification requirements for minority and women owned businesses, Good Faith Efforts, maintenance of a bidders list, and contract administration requirements. The DBE program also requires information on the participation of minority and women owned businesses be reported to the EPA.
- Debarment/Suspension Certifications: Entities appearing on the Federal Government's Extended Parties List are prohibited from participating in the DWRF/SRF programs and all bidders are required to provide a signed copy of the enclosed certification form.
The required bolierplate, Required Standard Contract Language: Clean Water State Revolving Fund and Drinking Water Revolving Fund, is available at this link and in the tables below. For questions, SRF/DWRF/SWQIF Disadvantaged Business Enterprise Questions and Answers.
The tables below list a number of DWRF and CWSRF/SWQIF forms/guidance that you can view and print.

To view and/or print a document in Portable Document Format (PDF), Acrobat Reader is required (a free download).
Helpful Tips:
- Since the documents are constantly being updated it is recommended you do not dowload and save copies, but rather access this site when you need to use one of the listed documents.
- If you do not have Microsoft Word 97, use the PDF version of the document.
- Review the quality of printing of your document and do not submit a document where printing is incomplete, not formatted properly, or otherwise looks odd.
- At this time, documents which require a signature can not be returned via e-mail.
If you decide to "Open the file from its current location," do not be alarmed if a "Warning" dialog box pops up indicating, "The document you are opening contains macros or customizations." All of the forms contain macros and/or customizations from a trusted source. Therefore, please select to enable the macros to proceed with opening the document. You can then fill in the form and print or save it directly from the Revolving Loan Section web site.
