• Use the PDF version to complete your application by hand.

• Save the PDF to your computer, then open and print.

• The Word document version contains form fields and will calculate totals for you.  Use this version to complete the application on your computer.

• Save the document to your computer, then open to complete the application.

• Be sure to go to Tools | Options | Print and uncheck 'Update Fields' in the Printing Options.
