PERMIT APPLICATION FORMS TO CONTROL NUISANCE AQUATIC PLANT AND/OR ALGAE GROWTH

Long Form Application - EQP 2790 revised 02/06

DOC format (62 KB)  All applicants using the .doc format may electronically complete the boxes/fields within the application form, but shall not modify the application form in any other way.  Modified application forms are invalid.

-   PDF format (61 KB)  When printing this form, uncheck "Fit to Page" (Adobe Reader Version 4 or earlier) or "Shrink oversized pages to paper size" (Adobe Reader Version 5) in the Print dialogue box so that the application prints at the correct size.

-   PDF fill-in format  (284 KB)  You may not be able to scroll through this document until it is completely downloaded.  Earlier versions of Adobe Reader may not display or save this document correctly.  The latest version is available free from Adobe.  If this form cannot be saved after it opens, you can also save the file to your hard drive by right clicking on the link and selecting "Save Target As..."  You will not be able to save the information you enter in the form fields unless you upgrade from Adobe Reader to Adobe Acrobat.
