The following guidance pertains to reporting general facility information under the AWR Program.

Section 2 - Facility Identification
2.1 Facility ID Number – Each facility reporting under the AWR Program will be assigned a facility identification number by the DEQ. Facility ID Numbers should be located at the top left corner of your mailing label. If you are reporting for the first time under the AWR Program, or do not know your facility ID number, leave this section blank. The DEQ will assign a facility number for future submittals.
2.2 Facility or Establishment Name – The name of the facility or establishment for which the form was submitted. For purposes of AWR, a facility is generally considered to be all buildings or structures owned and operated by a business on a single piece of property. The facility may be one building located in an industrial park, or a large complex comprised of several buildings covering 100 acres of land. Generally, a business will submit one wastewater report for the entire facility.
2.3 Owner/Operator Name – The name of the owner or the business, or the operator responsible for preparing and submitting the wastewater report form. If desired, the name of a senior management official at a facility may be used instead of the aforementioned options. The mailing address should be entered for the reporting facility.
2.4 Name and Telephone Number of Technical Contact or Person Completing Form.
2.5 Nature of Your Enterprise or Operation – Briefly describe the nature of your enterprise or operation. For example, a specialized electroplating business may describe their operation as "batch anodizing of machined aluminum parts.
2.6 Standard Industrial Classification (SIC) Code(s) – The primary SIC code for your business type should be entered in this section. If there is a secondary SIC code associated with your business, enter that number as well. The SIC code system will be phased out in the coming year, and will be replaced by the North American Industrial Classification System (NAICS). The wastewater report form will be changed over accordingly to reflect NAICS code system for the subsequent reporting year.
2.7 Toxic Release Inventory (TRI) Program Identification Number – Enter the number assigned to your facility for purposes of the TRI Program. In most cases, one number is assigned to each facility location. One company may however have multiple facilities located throughout the state, yet have only one assigned TRI number.
2.8 NPDES Surface Water Discharge Permit Number – If your facility has an NPDES surface water discharge permit, enter it as appropriate. Otherwise, enter "Not Applicable" or "N/A" in the box. All facilities discharging wastewater to a surface water (river, lake, creek, stream, drain, etc.) of the state are required to obtain an NPDES permit.
2.9 State Groundwater Discharge Permit Number – If your facility has a groundwater discharge permit enter the permit number, otherwise enter "not applicable" or "N/A" in the box.
Facilities that discharge wastewater to the groundwater should have a groundwater discharge permit.
2.10 Hydrologic Unit Code(s) – Enter the appropriate Hydrologic Unit Code that correlates to the geographical location of the facility. USGS Hydrologic Unit Codes can be found using the search engine at http://cfpub.epa.gov/surf/locate/index.cfm
