Section 3: Certification

The following information pertains to the certification of filing status, either abbreviated or full report, and the certification of the accuracy of the completed report by the owner/senior management official.

 

Section 3- Certification

 

3.1 Check the box marked “YES” if you meet all the following conditions:

  1. Wastewater was discharged to a water of the state or to a sewer system;
  2. Critical materials were not used in or incidental to manufacturing processes in excess of their annual use thresholds (found on Table 1 Critical Materials Register), and;
  3. Critical materials were not discharged in wastewater, other than those discharges exempted pursuant to R299.9006 (3) and (4).

 

After checking the yes box, sign and date Section 3.2 and submit Part I as your abbreviated wastewater report form.

 

Check the box marked “NO” if you do not meet all these conditions.  Then proceed with filling out the rest of the wastewater report form.

 

3.2 A senior management official or owner/ operator must certify that all information included in the Wastewater Report is true, accurate, and complete to the best of his or her knowledge. The owner or senior management official must print their full name and official title, and then sign their name and enter the signing date.  Each report must contain an original signature. The certification statement applies to all the information supplied on the forms and should be signed only after the forms have been completed.
