Sewage Sludge (Biosolids)DMR Forms Agency: Environmental Quality
These are EPA Discharge Monitoring Reports (DMR) to be used by facilities that use surface disposal (sludge monofills), landfills, or incinerate their sewage sludge. Do not use the DMR form for Land Application. The appropriate DMR report is now to be submitted to DEQ instead of the US EPA. Download DMR Information
