Form name When to file

EQP 7200-1 Application for Permit to Drill, Deepen, and Operate a Well

Instructions:

Initial well permit application
EQP 7200-2 Survey Record of Well Location Submit with application
EQP 7200-4 Wellhead Blowout Control System form Submit with application
EQP 7200-14 Injection Well Data & Attachments For injection wells, submit with application
EQP 7200-18 Soil Erosion & Sedimentation Control Plan When applicable, submit with application
EQP 7500-3 Environmental Impact Assessment (EIA) When required, submit with application

Bond for conformance forms:

For conformance bonding
IRS form W-9 Request for Taxpayer Identification Number and Certification Submit with CD bond
EQP 7200-13 Well Permittee Organization Report When requested or as part of application
EQP 7200-7 Request for Transfer of Permit When there is a change in owner/operator

The drilling & completion and production & operating forms should be submitted by email to EGLE-GeologicalRecords@Michigan.gov.

If files exceed 20 MB, submit digital copies by mail on a USB flash drive or CDR/DVD-R to:

All log files shall be submitted in Log ASCII Standard (LAS) and Tag Image File (TIF) format. PDF files and paper copies may be accepted with prior approval by EGLE, GRMD. Files shall be named using the well permit number, well name, and log type. All files are to be submitted as single page TIF and/or single page PDF. logs are due within 30 days of conducting the logging run.

Well Elevations - No form; File as required by {R299.2369} Within 60 days of drilling completion
Electric or Radiation Logs - No form; File as required by {R299.2370} Within 30 days after running log(s)
Survey of Directionally Drilled Well - No form; File as required by {R299.2371} Within 30 days after drilling completion
Note: a well may not be produced until survey is filed
EQP 7200-10 Water Well Record (for Oil, Gas, or Mineral Well Operations) Within 30 days after water well completion
EQP 7200-12 Certification of Casing and Sealing of Surface Hole Within 30 days after drilling completion
EQP 7200-9 Mud Pit Plot Within 60 days after drilling completion
EQP 7130 Record of Well Completion Within 60 days of well completion
Core descriptions - No form; File record as required by {R299.2369} Within 60 days of drilling completion
EQP 7200-6 Application to Change Well Status and/or Plug and Abandon Before start of operation
EQP 7200-8 Record of Well Plugging or Change of Well Status Within 30 days after change/plugging
EQP 7200-20 Water Well Plugging Record (for Oil, Gas, or Mineral Well Operations) Within 30 days after plugging
EQP 7200-11 Annual Certification for Blowout Preventers, Accumulators, Pumps and Secondary Systems Annually or when BOP is modified
