Permit Amendment Request Form

A permittee may request minor revisions to an approved permit.  Any request that is outside the scope of the original project must be submitted as a new permit application using the long form application.

-  ANC Permit Amendment Request Form DOC format All permittees using the .doc format may electronically complete the boxes/fields within the form, but shall not modify the form in any other way.  Modified forms are invalid.
