Referrals for a home study are initiated by the agency with legal custody of the child. A referral packet should consist of the following and ALWAYS submitted to the ICPC Office in triplicate.
FIA 4332 (100A) – Interstate Compact Placement Request
Proper documentation to support the application includes:
· Cover letter (on State letterhead) that summarizes the youth’s current situation and the permanency plan for the child; why the placement is being sought rather than a placement in Michigan and specific concerns to be evaluated by the receiving state.
· Financial/Medical Plan – If the child is IV-E eligible send verification (FIA 352).
· Social History, current progress reports.
· Current court order showing child’s legal status.
· Birth Certificate, Social Security Number, School and Immunization Records.
· Recent psychological evaluations, psychiatric reports.
· Any other pertinent information.
The receiving state must meet minimum home study standards CFF 922-1, including criminal history and central registry checks.
Form FIA 4333 (100B) – Purpose:
· When placement is approved, the 100B are forwarded to the receiving state with the date of placement to initiate supervision of the placement.
· Termination of the ICPC compact.
Use the FIA 4332 and FIA 4333 for NON DELINQUENTS except Residential Treatment.
Delinquent Wards use the FIA 3034, 3038, 3040, 3939 forms, except for Residential Treatment. Billing – the local office is the authority for payments (see CFS 900 Series). Transportation arrangements are to be made through the Interstate Office Unit.
PRIORITY PLACEMENT (ICPC Regulation 7)
A court order requesting the priority home study may be issued when the proposed placement is the child’s parent, stepparent, grandparent, adult brother or sister, adult uncle or aunt, or the child’s guardian. The child qualifies for priority placement under Regulation 7, section 5a or section 5b of the ICPC if one or more of the special circumstances exists:
· The child is under two (2) years of age.
· The child is in an emergency shelter.
· The child has spent a substantial length of time in the home of the proposed placement recipient.
· The receiving state has been in possession of the completed ICPC 100A form for over 30 court days, but the sending state has not received a notice-determining placement.
