Instruction Manual for Child Placement Agency & Child Caring Institution Cost Schedule (DHS-573)

The primary purpose of the Cost Schedule (DHS-573) and its supporting forms is to document provider management and service program costs. This information is used by the Michigan Department of Human Services (DHS) to establish an equitable rate for purchased services. The Cost Schedule also provides detailed information which can be used for cost analyses, research and evaluation purposes.
