Online Application Instructions for One Stop

CHILD CARE: 

To open a home or facility for the care of children, utilze the online child care license application.

Online Family Child Care Home Application (1-6 children)

Online Group Child Care Home Application (7-12 children)

Online Child Care Center Application

In addition, please print and read the Licensing Rules for Family and Group Child Care Homes or the Licensing Rules for Child Care Centers and 1973 PA 116 prior to submitting your application. 

To open a home or facility for the care of adults or children under adult foster care or child welfare guidelines (This is not for renewing an existing license or registration), download and print an application form. 

ADULT FOSTER CARE: 

Due to a high volume of pending AFC applications and the Division's limited resources, an AFC applicant can expect a waiting period of approximately 6 to 12 months before an original AFC license may be issued. Waiting periods may vary depending on complaint activity, workloads of local health authorities and the Bureau of Fire Services, and the timeliness of applicant compliance.

Child Placing Agency Original Application Packet 

Child Caring Institution Original Applicaton Packet 

In addition, please print and read the Licensing Rules for Child Placing Agencies and the Licensing Rules for Child Caring Institutions and 1973 PA 116 prior to submitting your application. 

CAMPS: 

To begin the application process, please submit the site and program applications, the required fees and the BCAL-1326 on the camp administrator.
