New forms MU1 and MU2, plus Michigan Specific Requirements will replace forms FIS-1018, FIS-1019, FIS-1020, and FIS-1021 in April 2008.  Applicants will use current forms until these new forms are placed on the website for use. Pending applications using current forms will be processed during this transition.
