Record Amendments-Other Information

1st and 2nd Mortgage License and Registration Record Amendments

First and Second Mortgage Licensee and Registrant Record Amendment Instructions

A licensee or registrant may amend information in its record when it wants to add or delete branch office locations, change its mailing address, telephone numbers, facsimile numbers, website addresses, or e-mail addresses.  It may, also, amend record information related to changes in company officials and ownership.

Branch Office Additions and Deletions

An office location where licensee or registrant conducts business with consumers, and is under the general control of the licensee or registrant, belonging to or leased by the licensee or registrant, and staffed with employees of the licensee or registrant, may amend the State of Michigan record by submitting page 4 of MU1 Form to the Consumer Finance Unit of the Office of Financial and Insurance Regulation.  There is no cost for this record amendment.

  • To add a branch office location, send page 4 of MU1 Form, identifying the request as other-record information, and completing item 1 (A) and (H).   Enclose attachments with other pertinent information related to each new branch(es), such as address location, telephone number, facsimile number, website address, e-mail addresses, and name under which it intends to operate.  The name of branch managers is included with this information.
  • To delete a branch office location, send page 4 of MU1 Form, identifying the request as other-record information, and completing item 1 (A) and (H).  Send a written request identifying the branch location, requesting removal from the record along with pertinent information related to the closing of this branch(es), such as where the records will be kept and maintained.

Mailing Address Changes

To request an amendment to the mailing address, if different from the main address, send page 4 of MU1 Form, identifying the request as other-record information, and completing item 1 (A) and (F).   There is no cost for this record amendment.

Amendments to Telephone Numbers, Fax Numbers, E-Mail Addresses, Websites

To request an amendment to the telephone numbers, fax numbers, website address, or e-mail address, send page 4 of MU1 Form, identifying the request as other-record information, and completing item 1 (A) and (G).   There is no cost for this record amendment.

Company Official and Ownership Information Amendment

Request an amendment to the executive officers, company officers, official managers, and control persons, by sending page 4 and Schedule C of MU1 Form, identifying the request as other-record information.   There is no cost for this record amendment.

If the amendment to ownership reported in Schedule C is greater than 25%, this change is defined as a transfer, which requires the Commissioner’s consent.  Select transfer information from the OFIR website to obtain those requirements.
