• All individual licensees use Address Change Request (ACR). A fee of $3.00 shall accompany each change of mailing address to cover costs of recording such change, and is collected by NIPR. The NAIC PDB must reflect an active resident license for the individual.
• Any non-resident adjuster must have only one home state license, or must use the FIS 0262 Name or Address Change for Insurance Licensees.
• Business entities must, and individual licensees may, use the FIS 0262 Name or Address Change for Insurance Licensees. A fee of $3.00 shall accompany each change of mailing address to cover costs of recording such change. Attach check or money order payable to the State of Michigan to the top of the completed form FIS 0262 and send it to the address on the form.
• Individual licensee residence address must each be a physical address. PO boxes are allowed for business and mailing addresses.
