| Appointment Procedure |

Either an individual producer
or agency, or both, may be an MGA.
An individual producer must be appointed by the company in order to be the
signer, even if the agency is also an MGA. The individual must be licensed as
a producer. To appoint the individual:
- Submit the form FIS
0242 MGA Agent Appointment/Cancellation. The individual must also be appointed
by the company as a producer/agent. The producer appointment must be made
electronically prior to the MGA appointment.
- This individual officer
must be associated with the agency (FIS
0200), if the agency is also appointed by the company to be the MGA.
If the agency is an MGA,
it must be licensed as a producer. To appoint an agency as an MGA:
- Submit the form FIS 0242 MGA Agency Appointment/Cancellation. The producer/agency must be
appointed by the company for the same qualification as the producer/agent.
The producer/agency appointment must be made electronically prior to the MGA
appointment.
- Submit the company/MGA
agreement/contract.
NOTE: More than one company
may appoint the same producer/agent and/or agency to be their MGA.
