APRIL 23, 1998 - The Bureau of Health Systems in the Department of Consumer & Industry Services today reported that an unexpected death occurred yesterday at the Harold and Grace Upjohn Community Center, a 130 bed nursing home in Kalamazoo.
The facility reported on April 22, 1998 that a resident was found wandering in her room with several injuries including bruises, a cut on her forehead and a sore knee. It appears that she may have fallen. She was sent to the hospital where she later died.
An investigation is being conducted by expert CIS nursing home staff, who had also been working with the facility due to recent problems investigators uncovered at the unannounced annual inspection conducted by the department on April 7, 1998. During that inspection the department assessed fines and ordered immediate actions to address problems in accident prevention.
The department is currently working with the Attorney General's office on a correction order that will allow CIS to impose further state penalties and remedies. The order should be finalized in the next couple of days.
In another nursing home death reported earlier in April, at a nursing home in Jackson, CIS investigators determined the death was due to bronchopneumonia and not to improper care.
