CIS Moves To Revoke License of Evergreen Manor Adult Foster Care Home

OCTOBER 14, 1998 - The Michigan Department of Consumer & Industry Services (CIS) has begun revocation proceedings against the Evergreen Manor adult foster care home in Bangor for violations of state law.

 

During the past four months, CIS Adult Foster Care (AFC) licensing consultants have investigated and substantiated 19 complaints at the facility.

 

"The violations we found at Evergreen Manor are of a severe and repetitive nature that we consider to be willful and substantial," said CIS Director Kathleen Wilbur. "Based on these findings, we are moving to revoke the facility's AFC license."

 

Many of the complaints target Evergreen Manor's failure to develop and implement specialized care and behavior plans. The facility also failed to properly notify the State about incidents that occurred there.

 

Under State statute, Evergreen Manor has 15 days to respond to the CIS licensing report.

 

"Based on the violations outlined in the report, it's license should be revoked," said Wilbur. "By law, the facility must be given an opportunity to respond. After the 15-day response period, a final determination will be made regarding the pending license revocation."
