New Hire / Rehire
This link is for employees who:

have never been employed by the State of Michigan, OR
- were previously employed by the State of Michigan but have separated
from employment.
Interdepartmental Transfer
This link is for employees who:
- are transferring to DLEG from another State of Michigan Department.
Student / Co-Op
This link is for employees who
- are new DLEG student assistants or co-op students.

ID Cards
To obtain a State of Michigan ID card a new employee must complete the DMB-624 form. For a regular ID card, the form must be signed by an authorized signer in the Office of Human Resources. For Access cards and keys, the form must be signed by an authorized signer from Finance - Office Services (Sharon Lycos or Leslie Christy).
- The replacement fee for lost Facility Access Key Cards and ID cards is $10.00 effective 3/13/08.
- Lost or damaged Facility Access Key Cards issued on or before October 1, 2003 are not subject
to the $10.00 replacement fee.
- Damanged ID or Facility Access Key Cards which are less than three (3) years from the date of
issuance are subject to the $10.00 replacement fee.
- There is no longer a charge for cards that have been damaged at work and need to be replaced.
