The Asbestos Program conducts on-site evaluations of the abatement activities of contractors and also responds to complaints or referrals involving improper work practices or procedures during asbestos abatement or disturbance activities. 

The following Notice of Alleged Safety and Health Hazards (complaint form) allows an employee to report hazards in the workplace to MIOSHA. It has been designed to be completed, printed, then either faxed or mailed, or to be completed and submitted online.
