Except as otherwise provided by PA 48, a provider shall file an application for a permit (see MPSC web site for sample "safe harbor" permit and application forms) and pay a one time $500 application fee to each municipality whose boundaries include public rights-of-way for which access or use is sought by the provider (Sec. 6 (4)). Applications must include route maps showing the location of the provider's existing and proposed facilities in electronic format (For more information on route maps, refer to the METRO authority Route Map Minimum Requirement Instructions/Guidelines). This information is exempt from FOIA, unless a mandatory protective order has been issued by the MPSC.
Within 90 days after the substantial completion of construction of new facilities in a municipality, a provider must submit route maps showing the location of the telecommunication facilities to both the commission and the affected municipalities. Again, this is required in electronic format, as prescribed by the MPSC (Sec. 6(7)).
