The department may reject a report filed by a resident trainee, if the report is inaccurate, incomplete, or if it is filed after the due date. A trainee is required to report at least 5 embalmings during each six month reporting period. Each report must be certified by the resident trainee's sponsor.

SPONSOR REQUIREMENTS

A mortuary science licensee may be eligible to be a resident trainee sponsor after five years of licensure. Only one trainee may be sponsored at a time.

CHANGE OF SPONSOR

A resident trainee who changes sponsors during his/her resident training period is required to submit a Change of Sponsor form to the department. The trainee should also submit an Embalming and Resident Trainee Report signed by the trainee's previous sponsor, summarizing the work performed between the last report and the date of the change in sponsor. Failure to submit the report may result in a loss of training time and services.
