Ocularists whose registration has been lapsed for 60 days past the expiration date of the license must apply for re-registration. Forms may be obtained by calling the Board office at (517) 241-1306, or by e-mail providing a mailing address.

Ocularists whose registration has been lapsed for more than 3 years are required to meet current registration requirements. Questions may be directed to the board office in writing, by telephone or by e-mail.
