(1)   New Applications received between 05-04-2009 and 9-30-2010 pay the two-year fee; New Applications received between 10-01-2010 and 5-3-2011 require the one-year fee.  Registrations expire on 9-30 of the odd-numbered years.

(2)   Reregistration fee (application submitted for reregistration that is received more than 60 days after registration expires) is comprised of two per-year fees, an application processing fee and a late fee.

(3)  A reinstatement application is submitted with the application processing fee only and is paid with an application to become reregistered after the registration was revoked.
