How do I obtain the Quick and Rapid Copy order forms?
 
Answer:
Quick Copy and Rapid Copy Order forms are available at all copy centers. In addition, Rapid Copy Order forms (DMB-551) may be ordered from DMB, Materials Management Services, using commodity number 4828-1882. These forms are shipped in bundles of 25 per package.
