I cannot find the profile form on the Internet, how else can I obtain it?
 
Answer:
It is necessary to revise the form from time to time.  When this occurs, the form is taken off of the website until revisions are finalized.  You may call Ronda Whiting at 517-322-5110 and one can be faxed to you.
