When filling out a DMB-47, what information do I need to have?
 
Answer:

When filling out a DMB-47 you will need to fill in all applicable information. You must also include receipts when submitting your DMB-47. If your DMB-47 is missing any of the following information, it will be returned to you for completion:

 

1.  6 digit vehicle number.

2.  Both the driver and the supervisor's signatures.

3.  Social security number.
