DTMB Facilities Security Clearance Request, Project-Related Contractor Access

Contractor Procedures for Obtaining Access to DTMB-managed facilities:

Access the DTMB Security Clearance Form (DTMB-453). Review the instructions and complete the form.
Submit the completed form to DTMB Security via:
- e-mail to DTMB-Security@michigan.gov
- U.S. Mail to: DTMB Security & Emergency Management, Joint Operations Center, 615 W. Allegan Street, Lansing, Michigan 48909
The approved DTMB Security Clearance Form will be valid for one year from the date of receipt unless otherwise indicated.
The contractor is responsible for updating and maintaining the form.
