FAQ
  Military Records
How do I request a military record?

Answer:

All requests for military records must be in writing and include the following information about the service member:

  • Complete name as used during the time of service
  • Service number (if used instead of Social Security number)
  • Social Security number (even if a service number was used)
  • Branch of service (Army, Air Force, Navy, Marines, or Coast Guard)
  • Dates of service
  • Birth date (if the service number and/or Social Security number are not known)

Although not required, Standard Form 180, Request Pertaining to Military Records, is an easy way to make sure your request has all of the necessary information. Send requests for military records to the address on the form or:
