Complete an Electronic Funds Transfer (Direct Deposit) Application form and mail it to:

Military Affairs Retirement System

Michigan Department of Military and Veterans Affairs

Federal Retirement Benefits

 

Complete a Direct Deposit Authorization (SF 1199A) form and mail it to:

Retired Pay Operations (PPO)
DFAS-Cleveland Center
