Michigan Historical Marker Application Forms

Please read the following carefully, so that you download the application that is appropriate for the site you wish to designate via the Michigan Historical Commission's Michigan Historical Marker Program.

Currently Registered Sites:

If the resource you wish to mark is already listed in the State Register of Historic Sites or individually in the National Register of Historic Places, use the following application in the format you desire:

Not sure if your resource is registered? Search the Historic Sites Online database.

Sites not yet registered:

If the resource is not designated at the state or national level, use the following application in the format you desire: If the new site that you wish to designate is a cemetery, you must also complete the following form (use the format you desire): Commemorative markers

If you wish to mark a location without aboveground historic resources that is associated with an important historical figure, organization, or event, or if you wish to commemorate a national register district or an archaeological site, use the following application in the format you desire: Contact the Michigan Historical Commission.
