Calculation Data for Schools and Libraries
In an effort to assist Michigan
USF applicants and trainers obtain National School Lunch Program (NSLP) data
necessary for the calculation of their USF discounts, the Michigan Department
of Education prepares two worksheet documents each year. These calculation tables
(worksheet documents) provide the NSLP data necessary for determining the USF
discount for a school/school district or library/library consortia. This information
may be helpful in completing Form 471, Block 4, Item 10c. The tables may also
be of value to you in providing the required discount calculation documentation.

A school/school district
worksheet is available which provides NSLP data and the related USF discount
percentage for each school listed on the worksheet. In addition, the weighted
average discount calculation is listed for each school district. The weighted
average discount calculation is based on the assumption that all schools in
a school district are recipients of the shared service identified in Form 471,
Item 21.
Depending on each circumstance,
it may be necessary to adjust this information to accurately reflect your situation
as illustrated in the following examples:
1) If all schools located
in a school district are not participating in "shared services," then
the calculations would need to be adjusted to reflect only those schools participating
in shared services.
2) If a school district is filing for eligible services provided to an administrative
building as part of the network of shared services, then the building would
need to be added to Form 471, Item 10b.
3) If the reported NSLP data for a school contains pre-K students (who are ineligible
under the program rules), then the reported number of NSLP students listed in
the worksheet would need to be adjusted.
There may be other circumstances unique to your area that should be identified
and reflected in your reporting. In all cases, the applicant should carefully
follow the official Form 471 instructions and program eligibility requirements.
