It is important to note that the data contained in the 2003 (Year 6) worksheets is based on enrollments and participation reported to the
Michigan Department of Education in October 2001. You are allowed to use more recent data if it is available. Also, you may use eligibility
data if it is available. Please keep in mind that the information provided in Block 4, Item 10c is subject to program audit and must be documented.

The information provided in the following worksheets is arranged in alphabetical order by school district. The school district name and
weighted average discount follows a listing of the schools within the district
