- PLEASE--using an appropriate marker (like a Sharpie) or pen--legibly note on the lid and label the date of collection, producer's last name and/or farm name, animal's identification (ID), tissues collected, sex and age. Be sure to write MDA-CWD on the lid for billing purposes.
- Identification – Please prefix the animals ID using the letter “C” for Cull, “E” for Euthanasia or “D” for Death loss. If the animal has
an USDA tag number then place the appropriate prefix at the beginning, i.e., C34DMH0402. If the animal doesn't have an official USDA tag, then use whatever ID being used (present or previous owners) and the appropriate prefix or if no ID then use any that will help identify to premise of origin along with the appropriate prefix (examples – CRDH01, D01, ERoo2).
