Apply for Child Development and Care Assistance

To apply now, follow these steps:

If you plan to use an unlicensed child care provider who is not currently enrolled by the Department of Human Services:     

Have your provider mail or take pages 1, 2 and 3 of the completed form to your local DHS office as soon as possible. Your provider should keep page 4.

Contact your local Department of Human Services (DHS) office if you need a printed Child Development and Care application. You can find the telephone number and address of your local DHS office in the DHS County Offices information page.
