The following application is required for all projects submitted to the SHPO for Section 106 review, unless the project is a housing rehabilitation. The accompanying instructions provide detailed information to assist you in completing the form. Projects involving cell towers should follow the guidelines described in the document below:
USGS topographic maps, required for the review process, are available through the USGS website and through private vendors.
For projects requiring consultations with federally-recognized tribes in Michigan, contact the tribe in question. Each of the following tribes also has a Tribal Historic Preservation Officer (THPO):

Please respond to each point in the application, even if there is no information available. The terms "not applicable" or "unknown" are not acceptable responses. Please also note that SHPO cannot accept requests for Section 106 review via fax or email at this time. Fax submissions are often not clear enough for the reviewer to see the necessary detail and State of Michigan email limits the size of incoming file attachments.
