Changing a Currently Approved Library Technology Plan

Send via mail or fax a copy of your technology plan approval page (the first page of the application form that we have sent back to you with a signature) along with your suggested revision(s) and/or addition(s). Make a cover note explaining that you would like to either revise or add to your existing approved plan.  Please include your e-mail address, as you will receive an approval for these changes via e-mail.  We will copy this approval communication to your file at the Library of Michigan. If you prefer your approval via fax or mail as opposed to e-mail, please indicate this in your cover note. Keep all of your correspondence and approval documentation for your files in case of audit.
