Tips for Using the PDF Forms

You may complete the forms in any of three ways:

  1. Print the form directly from your browser. Then type or fill it in by hand (please print). Sign and date the form, make necessary copies and place in the mail.

  2. Save the form to a file. Fill it in at your computer after you leave the Internet. If you have set up your browser to open Acrobat Reader within the browser window, you have access to the browser's "File -> Save As" feature. Save the form to a folder on your computer; go off-line; open Acrobat Reader as a separate program, then open and complete the form. You will be able to print, but not save, the completed form. Sign and date the form, make necessary copies and place in the mail.

  3. Fill in the form while on-line, then print it.

    • Note that you risk losing your work if your browser crashes or your internet connection fails. (If you have set up your browser to open Acrobat Reader within the browser window, you have access to the browser's "File -> Save As" feature and can also save your completed form. It can then be reopened in the Acrobat Reader program for future reference.)

    • Completing the form on your computer:

      • The "hand" cursor will turn into a text tool when placed over a field in which you can type. Click in the first text box with your mouse, type your information, then tab to the next field. To return to a field later, click in the field or use shift-tab to tab backwards. Click in a check box to put an X in it.

      • The type size is small to allow you to include as much information as necessary. To ease viewing while completing the form you can enlarge the form by clicking on it with Acrobat's magnifying glass.

      • When printing, check the "Shrink to Fit" option on the Print Screen to optimize the form for your printer.

    • Forms cannot be sent electronically. Print, sign and date the form, make necessary copies and place in the mail.
