INFORMATIONAL STATEMENT AND FORMS WITH INSTRUCTIONS
ISSUED PURSUANT TO ACT 306 OF THE PUBLIC ACTS OF 1969 AS AMENDED
In the interest of promoting efficiency and enhancing availability
of insurance in this state, the Michigan Insurance Bureau has adopted
the Uniform Certificate of Authority Application (UCAA) developed
by the National Association of Insurance Commissioners (NAIC) for
use by all insurers applying to do business in Michigan as either
authorized insurers or eligible surplus lines insurers. Persons planning
to form a Michigan domestic insurer should use the UCAA-Primary Application.
All others should use the UCAA-Expansion Application. Both UCAA application
forms can be obtained from the NAIC or downloaded from the NAIC's
Web site: www.naic.org/ucaa/.
State specific components can be obtained from the Insurance Bureau
or from the Bureau's web
page. The Insurance Bureau will also accept applications prepared
on the previous application forms from companies which had requested
them prior to the effective date of this bulletin.
