Michigan law (MCL 500.1238(1)) requires any agent, solicitor, counselor, or adjuster to notify the commissioner of any change in his or her mailing address within 30 days of such change.
All licensees, with the exception of business entities, use the Address Change Request (ACR) gateway on the NIPR website at www.nipr.com. A fee of $3.00 shall accompany each change of mailing address to cover costs of recording such change, and is collected by NIPR. The NAIC PDB must reflect an active resident license for the individual in order to utilize ACR.
All non-resident adjusters must have only one home state license to utilize ACR.
Business entities must use the FIS-0262 Name or Address Change form. A fee of $3.00 shall accompany each change of mailing address to cover costs of recording such change. Attach check or money order payable to the State of Michigan to the top of the completed form FIS 0262 and send it to the address on the form.
Individual licensee residence address must be a physical address. PO boxes are allowed for business and mailing addresses.
A new license is not automatically generated when an address change is processed.
