To comply with this requirement, the Bureau of Health Professions frequently publishes a Disciplinary Action Report (see below). The Disciplinary Action Report (DAR) lists the disciplinary actions taken against health professionals who are licensed and regulated by the various health boards within the Bureau of Health Professions. The report also includes updated information regarding licensees who have appealed the Board's action to a higher court. The report includes the names of the health professional, their professional license number, the type of disciplinary action taken, the effective date of the action and the general nature of the complaint.

To obtain a copy of administrative documents pertaining to a specific health professional or to obtain copies of previous Disciplinary Action Reports write to: Freedom of Information Coordinator, Department of Licensing and Regulatory Affairs, Bureau of Health Professions, PO Box 30670, Lansing, MI 48909. You may also fax your request to (517) 241-1212.  Note: Please submit your request to us in only one format. 

Past Disciplinary Action Reports: Electronic copies of reports from 2006-2011 are available at http://worldcat.org/oclc/70910395/viewonline). Print copies of reports from 1993-2005 are available for review at the Library of Michigan at www.michigan.gov/mde/0,1607,7-140-54504---,00.html.  

Final administrative disciplinary action is subject to judicial review.
