Optometry Exam Information

In order to become licensed in Michigan, an applicant must submit an application to the Department of Licensing and Regulatory Affairs, Bureau of Health Professions. An application packet can be requested by email at bhpinfo@michigan.gov   or by calling (517) 335-0918.

An applicant must have graduated from a college or school of optometry that is approved by the board. Applicants must submit passing scores on Parts 1, 2 and 3 of the National Board (NBEO) Examinations. These scores must be sent to our office directly from the National Board.

Applicants for a Michigan Optometry license are also required to complete the Michigan Optometry Laws and Rules exam that is part of the application packet. The examination must be completed and returned to the Michigan Board of Optometry before a license is issued.
