An "ambulatory surgical center" is a Medicare-certified facility which operates exclusively for the purpose of providing surgical services to patients not requiring hospitalization.
Comprehensive Outpatient Rehabilitation Facility (CORF); L&C: 517/241-3830
A "comprehensive outpatient rehabilitation facility" is a nonresidential facility established and operated exclusively for the purpose of providing diagnostic, therapeutic and restorative services to outpatients by or under the supervision of a physician. At a minimum, the CORF must provide physician services, physical therapy and social or psychological services.

A "clinical laboratory" is any facility which examines human blood, tissue and fluid specimens for the diagnosis, prevention, or treatment of any disease or impairment of, or the assessment of the health of, human beings.

An "end stage renal disease facility" is a Medicare certified supplier of renal disease services, which may include rental transplants, dialysis, and various diagnostic, therapeutic and rehabilitative services.
Freestanding Surgical Outpatient Facility (FSOF); L&C: 517/241-4160
A "freestanding surgical outpatient facility" is a facility, other than the office of a physician, dentist, podiatrist, or other private practice office, offering surgical procedure and related care that, in the opinion of the attending physician, can be safely performed without requiring over night inpatient hospital care and is not operated as part of a hospital. It does not include a surgical outpatient facility owned by and operated as part of a hospital.

A "freestanding surgical outpatient facility/ambulatory surgical center" is a facility that is both licensed as a FSOF and certified as an ASC.
Home Health Agency

A" Home Health Agency" is an organization primarily engaged in providing skilled nursing services and other therapeutic services to patients in their own homes.
Hospice

A "hospice" is a health care program that provides a coordinated set of services rendered at home or in outpatient or institutional settings for individuals suffering from a disease or condition with a terminal prognosis.

A "hospital" is a facility offering inpatient, overnight care, and services for observation, diagnosis, and active treatment of an individual with a medical, surgical, obstetric, chronic, or rehabilitative condition requiring the daily director or supervision of a physician.
Outpatient Physical Therapy/Occupational Therapy and/or Speech Pathology Services (OPT); L&C:
517/241-3830 An "outpatient physical therapy/occupational therapy and/or speech pathology services" program is a rehabilitation agency, clinic or public health agency which provides outpatient physical therapy, occupational therapy and/or speech pathology services, designed by qualified personnel to upgrade physical functions.
Portable X-Ray Services (X-RAY); L&C: 517/241-3830
"Portable x-ray services" refers to a supplier of diagnostic x-ray services furnished in a place or residence used as a patient's home.
Psychiatric Hospital and Unit; L&C: 517/241-3841
A "psychiatric hospital" or "psychiatric unit" offers inpatient services for observation, diagnosis, active treatment, and overnight care of persons with a mental disease, or with a chronic mental disease or condition requiring the daily direction or supervision of physicians licensed to practice in the state. The term does not include hospitals and programs licensed or operated by the Department of Community Health or the Department of Corrections (see "Facilities Not Covered by BHS" below).
Rural Health Clinic (RHC); L&C: 517/241-4160
A "rural health clinic" is a Medicare certified clinic located in a rural area having a shortage of health manpower, a shortage of personal health services, or high migrant worker impact. These clinics are typically staffed by a physician assistant supervised by a physician and may include mobile units.

Adult Foster Care homes are residential settings that provide 24-hour personal care, protection, and supervision for individuals who are developmentally disabled, mentally ill, physically handicapped or aged who cannot live alone but who do not need continuous nursing care.

A supervised personal care facility that provides room, board, and supervised personal care to 21 or more unrelated, nontransient individuals that are 60 years of age or older.
